Of quervain disease: symptoms, diagnosis and treatment

Quervain disease is a disorder of the tendons of the thumb as they pass through the articulation of the wrist and attached to the bone of the thumb. While crossing the wrist joint, they pass through a tunnel of fibrous tissue, called a Retinaculum. Here, they are lubricated by a synovial sheath called tenosynovium.

In this condition, the tenosynovium loses its lubrication and tightening tendons. This causes pain in different activities that involve the thumb.

Different conditions that cause the disease include over userepeated soft injurygoutrheumatoid arthritis

Symptoms include pain located just behind the base of the thumb. Pain is enhanced through activities that involve the movement of the thumb. Slight swelling may be present on the site of pain. Tenderness is present on the tendons where they spend the articulation of the wrist (radial styloids).

This disease is 10 times more common in females and is seen usually between the ages of 35 and 65.

Diagnosis is made by history and physical signs. the rays of the wrist are usually normal.

Treatment includes the following

Most people get complete relief after a single injection of steroids in the sheath of the tendon. Very few patients require surgery. During the operation the fibrous tendon tunnel is open. This frees their constriction and provides relief of pain.
